Please Bring With You:
1.       Insurance card
2.       Valid photo ID
3.       Co-payment (cash, check, MC, Visa, Discover accepted) as noted on your insurance card
4.       A complete list of all medications you are currently taking (please include medication name, dosage, refill date, and pharmacy).
Registration and Health History Forms (required before appointment):
The preferred way to complete forms is through our website’s secure portal. If you are unable to complete forms online, see other options (below). 
  • Option 1 (preferred): Complete forms online through our secure portal. Follow the instructions below.
    Click here to Create an Account. Remember your user ID and password so you can access your account        in the future to obtain lab results, request a med refill, or ask a question.
    b.     Select a location. Women’s Wellness Center or Center for Maternal-Fetal Care
    c.      Select a provider. Open and complete each form indicated for your selected provider.
    d.     **IMPORTANT** When completing the Personal Health History Form, be sure to click NEXT at the bottom        of each page and SUBMIT at the end to ensure we are able to retrieve your information.
    e.     If you have difficulty using the portal or completing forms online, please click here to report details of        problems encountered – this will allow us to improve our process – thank you for taking the time!
    f.       If unable to complete forms online, proceed with option 2 or 3 below.  
  • Option 2: Mail, fax, or bring completed paper forms. To access paper forms, Click here to reach our Forms page. Follow instructions:
    a.    Choose the appropriate forms, print, complete, and bring at least 15 minutes before your appointment (or       fax/mail so we receive them at least 2 days in advance).
    b.    Which forms do you need to complete?      
            ●    All new patients should complete a Registration Form and a Personal Health History  
            ●    If you are visiting a specialty provider (acupuncture, psychologist, health coach, or
                  massage) please also complete the appropriate specialty services form.
    c.     If bringing completed forms with you on appointment day, please arrive 15 minutes early to allow our staff       to create your chart and enter data from your forms into your electronic health record. 
  • Option 3: Arrive early and complete forms here.
    a.    If you plan to complete forms on the appointment date, please arrive 20-30 minutes early to allow time for       you to complete the forms and our staff to create your chart and enter data.
    b.    You must have your paperwork completed in advance of your appointment time, as the time scheduled for       your appointment is meant to be the time you are ready to see the provider.

On-Time Arrival Policy

  • Please arrive on time (arrive early if you did not complete forms online – see above).
  • Our Policy: If you arrive more than 10 minutes after your appointment time, you may need to be rescheduled. 

Cancellation Policy

  • Please give us the courtesy of calling at least 24 hours in advance if you need to cancel an appointment.
  • Our policy: If you no-show or late-cancel 3 times, you may be dismissed from the practice.

Do I need to send my medical records before my visit?
* In most cases we do not need your complete medical records in advance. Please follow these guidelines:

1. If you have a new appointment with a WWC family medicine physician, it would be helpful to send your immunization history before the first visit. To send these records, print a Release of Records Form, complete, and present to the applicable health care provider, so they can send records to WWC before your appointment (or you may bring immunization records with you).

2. If there is a specific health issue you believe your WWC provider will need to review, please have the appropriate office send records pertaining only to the specific health issues of concern (rather than the complete medical record). * Why do we not encourage patients to provide their entire medical record? Because, it results in waste of paper and time. In most cases only selected items are pertinent for your new physician to review. They will determine what records they need (if any) during your visit.
